Procedure of applying First Aid / Home Nursing


The applicant should send in the course enrolment form to Secretariat Office (via email: firstaid@sjas.org.sg) to inform that you are organizing a course for your unit; you have to specify who is the lecturer for the first aid classes and registered nurse (attach the SRN practice certificate as a supporting document) for the home nursing classes in the form.

For home nursing examiner, please send in the SRN practice certificate as a supporting document.

After completed the examination, you have to submit a soft copy of the nominal roll (via email: firstaid@sjas.org.sg) and hard copy of the nominal roll and examiner's report with the respective signatory to Secretariat Office within one month. With all the proper documentation, we will process the certificate and send it to the Association Director for his signatory. This process will take about 1 to 2 weeks.

With the above arrangement, it will speed up the process of the certificate for Brigade members.

Below is the instruction when you fill in the nominal roll:

All entries are to be printed IN BLOCK CAPITALS.

All participants are to ensure that their NAMES and NRIC Numbers are CORRECT before signing the attendance record.

The Secretariat Office will replace certificates for typo errors made by the office.

The Secretariat Office is not responsible for errors in the certificates if the participants failed to check for the correctness of

their names and NRIC Nos. in the attendance record.  No replacement certificates will be issued.

A soft copy of this attendance record shall be sent to firstaid@sjas.org.sg follow by a hard copy to the Secretariat Office.

This form and the Examiner's Report shall be submitted to the Secretariat Office within ONE month upon completion of the examination.

The Secretariat Office will reject this form and examiners' reports if the submission is more than ONE month after the examination date.

The Secretariat Office will process the certificates within 1 - 2 weeks after verifying all entries are CORRECT.

Footnote: Number of participant per class is 25.

Certification:
A first-aid certificate (valid for 3 years) shall be issued to each and every successful candidate.

Course participants are required to provide correct personal particulars upon enrolling for the course. If these particulars are incorrect and a replacement certificate is required, then a replacement fee of $10.00 shall be levied.
